Owner-only SMS development test consent

This planned program uses fictional service data and the owner’s authorized personal phone to test SMS delivery reporting, inbound messages, one human response through Spearis Inbox, HELP/STOP handling and duplicate prevention. Testing begins only after the required provider approval, secure configuration and separate test authorization. This program is not open for customer enrollment.

Select each purpose you authorize. All boxes start unchecked:

[ ] One human-written Spearis Inbox response to my test-phone message.

I expressly consent to the selected SMS test purposes at the number above. Consent is voluntary and is not a condition of purchase. My number must be restricted in the test configuration before any send. Signing records consent only; permitted sends still require the separate controlled-test authorization.

Frequency: Up to two application-originated messages in this checkpoint: one sender-test message using one selected service template, plus one inbox reply. This is not one sender test per service. Additional provider-generated responses may follow participant-initiated HELP, STOP or START messages. No recurring customer enrollment is authorized.

Message and data rates may apply. Reply HELP for help or contact spearismarketing@gmail.com. Reply STOP to opt out. START does not automatically remove Spearis’s suppression record. Carriers are not liable for delayed or undelivered messages.

Retain the signed original privately. The publicly accessible evidence copy is a blank form; do not publish your personal phone number or signature.
